What Is the GUIDE Program?
GUIDE stands for Guiding an Improved Dementia Experience. It’s a Medicare model created for those living with dementia as well as the unpaid caregivers who support them. The program is designed to give families more organized guidance, practical resources and steadier support as care needs change.
Built Around Three Goals
The GUIDE Program is focused on three things that matter most to the families we serve.
A better quality of life.
Dementia care should support the whole person, not just the diagnosis. GUIDE helps address behavioral health needs, functional needs, medical coordination and everyday care with the compassion every family deserves.
Less strain on caregivers.
Caregivers carry a lot, often quietly. GUIDE offers caregiver skills training, support services, access to a care team, connections to community resources and respite care, so caregivers don’t have to do it all alone.
More time at home.
With the right support in place, many families can help their loved one remain safely at home longer. GUIDE is designed to help prevent or delay long-term nursing home care for as long as it’s appropriate.

Do You Qualify?
Your loved one may be eligible for GUIDE support through Elite if they:
- Have a dementia diagnosis
- Have Medicare Parts A and B as their primary coverage
- Are not enrolled in a Medicare Advantage plan
- Are not enrolled in Medicare hospice or PACE
- Are not currently living in a long-term nursing home
